An AARP survey conducted in 2006 found that nearly 90% of people aged 50 and older want to stay in their homes as long as possible. More than 80% who receive long term care receive it in their home.

TheraCare  In-Home Caregiving was founded by a Physical Therapist who worked extensively with patients as they prepared to return home after a stay in the hospital or skilled nursing facility. Time and again, he noticed a void in the coordination of care for those returning home. He wanted to offer a more customized home care solution incorporating my medical experience with a personal touch to ease each client’s transition home. Our mission at TheraCare is to provide safe, customized home care services with a therapeutic touch.

TheraCare clients can expect:

  • A Home Safety Assessment performed by a physical therapist and certified aging in place specialist

With 15+ years of clinical experience, TheraCare staff has a unique advantage in assessing each client’s needs. We ensure that the initial plan set forth by the physical and occupational therapists are properly carried out. If no plan is in place, we will establish one. We work hard to match the right caregiver with each client to create a good fit. In addition, our CAPS specialist (Certified Aging in Place Specialist) will personally assess each home for safety to make sure both the client and the caregiver are functioning in optimal conditions.

Logistics:

  • No care minimum, up to 24 hour care
  • Personal Care Attendants are employees of TheraCare – Liability Insurance and Worker’s Compensation is provided by TheraCare
  • A complimentary, no-pressure assessment by a Physical Therapist is conducted before the start of care
  • Every caregiver is trained, at the client’s home, with the client, before starting any new case
  • Dedicated Nurse Case Manager to oversee scheduling, implement care plans, and do home visits to ensure quality of care
  • Coordinate with Long-Term Care Insurance policies

Our personal care attendants are carefully hand picked. Background and reference checks are performed. Prior work experience and skills are validated by our physical therapist and nurse case managers. An extensive training program is in place and each caregiver receives training on spotting and reporting Elder Abuse. In addition, each personal care attendant, if an employee of TheraCare and is covered by professional liability, bonding, and workers compensation insurance.
